We need 720 petitions filled out and signed by registered
voters of district 95 inorder to have Scott Yardley’s name
listed on the 2010 ballot.

95? It’s on your voters card. If that is not
Finder. After you fill out and submit the
form it will show your precinct. If you
click the “Continue” button, you’ll see a
“My Districts” link. Click on that link and
look for the “State House District” line. If
it’s 95, please print this form, fill it out and
mail it to us.
Please take your time and fill out the form
very clearly. Illegible forms are tossed out
by the Supervisor of Election.
Sign your name the same way you did
when you registred to vote. Miss-matched
signatures are also tossed
You don’t need to fill out both your birth
date and voters registration number. Just
enter one or the other. Your voter number
is on your voters ID card.
